The username/password box is not displayed when I attempt to login to my account.

If you are using Internet Explorer, make sure you have not customised the logon setup in the options.

Internet Options -> Security Tab -> Customise -> User Authentication -> Logon is set to "Automatic logon in Intranet zone only" by default.

If you have changed this to "Anonymous Login" you wont see the username/password prompt and you will not be able to access your account. Just change this setting to the default and you should see the dialog. Close down your browser and restart if you continue to have trouble.
